For fields of type Single Select, Radio Button, Checklist, and Multiple Select an option to include "Other" that provides a textbox for response would be very useful.

I happen to always forget to assign the proper Form Access permissions in Account Settings. I'm wondering if it could become part of the Form Setup process. Form Access could be a link under Form Settings.
